How do I?

Change of Patient's Personal Details

Find my NHS Number

Your NHS number is a 10-digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

You can probably find it on a document sent to you from the NHS like a prescription or appointment letter.

If you can’t find it, you can use this online NHS service. They will ask for your name, date of birth and postcode. They will send a text, email or letter with your NHS number to you.

Set up Proxy Access (Linked Profile)

As a parent, family member or carer, you may be able to manage services for another person by switching to their profile. This is also sometimes called having a linked profile or having proxy access. It needs to be set up by us at the GP surgery. You and the other person will both need to be registered with us. You can then switch profiles using the NHS App.

To set this up you need to provide identification. This is so we can confirm who you are and check it’s appropriate for you to act for the other person.

Depending on the level of access we agree to give you, you may be able to switch profiles and:

  • book an appointment for the other person
  • order a repeat prescription for them
  • view some or all of their GP health record

Contact us (preferably by email sxicb-wsx.selsey-selsey@nhs.net ) with you request if you would like to organise this service.
